Already Receiving Care Through the VA? Here’s How to Switch to Whole Life

If you are currently receiving VA-funded home care through another agency but would prefer to work with Whole Life Home Care, follow the steps below

Let your VA Primary Care Team or Social Worker know that you’d like to switch your Homemaker/Home Health Aide (HHA) services to Whole Life Home Care.

You don’t need to give a reason unless asked, but feel free to share what you’re looking for (better communication, consistency, local ownership, etc.).
Ask them to update your HHA referral to reflect your preference for Whole Life Home Care (NPI: 1376250464).

Whole Life Home Care is an authorized provider with the VA.

You can mention this to your care team to ensure there are no delays in processing the switch.
You are not required to explain your choice to them, but it’s courteous to let them know once your referral has been officially changed.
The VA will handle coordination from there.

Once you’ve requested the change, give us a call so we can:

  • Confirm your referral has been updated
  • Coordinate with your VA care team
  • Begin onboarding and assigning a caregiver
